Will there always be a clinical team member available to take my call if I have an urgent enquiry outside of regular scheduled contact?
How many phone calls per year will I receive from a member of the team?
Under the Plan you can expect a minimum of two calls per year in addition to your face-to-face consultations. Our specialist support team will also be in contact on a regular basis to make the arrangements for your care.
Will my insurance company cover the package?
This will depend on your insurance policy and what it covers with respect to your condition. We recommend you always first check and obtain authorisation from your provider if you intend to claim for the Care Plan.

The Flash Glucose Monitoring System is simple to use, discreet, and best of all it doesn’t need fingerpricks! Attached to your upper arm, it not only gives you your glucose values, but also shows the current trend (whether it is going up or down) as well as the trends of the past 8 hours. You can download all data (up to 90 days) to gain insight into your blood glucose trends over an extended period and to assess how your lifestyle and medication can contribute to those trends. It is perfect for a busy lifestyle where you might not be able to measure all the time. This also frees you up to concentrate on other aspects of your life.
What are the price comparisons if I paid for these services individually?
Depending on your specific care package, the Care Plan represents savings of at least 25%.
Can I pick and choose only those services of interest to me within the Plan?
The Plan is designed to provide a core level of required services suitable to your particular needs and this will be discussed with you by your consultant. Additional services can be added to your Plan and these are easily arranged for you by the team. You may be charged for these additional services.
What happens if I wish to cancel or don’t use all of the services included in the Plan?
The Care Plan has a minimum contract term of two years. We will ensure we deliver the service to you as described, offering very flexible appointments and delivery mechanisms to ensure that you receive the benefits of the Plan. You can cancel the Plan by writing to us at any time, but you will still be liable for any outstanding amounts under the Care Plan agreement.
I’m type 1, why can’t I go on the Plan?

Will my GP have access to my notes?
When you register we will request contact details for your General Practitioner. As a matter of good practice we will write to your doctor to keep them informed of your care and results. On commencing the Plan, please make the team aware should you not wish for this to happen.
Does the Care Plan include counselling services for newly diagnosed patients?
Although this is not covered in the Plan, we do have counselling professionals (clinical psychologists) available for appointments at a discounted rate to Care Plan members. Counselling services are at an additional cost to the Plan.
I’m an overseas patient and I only see a consultant once
a year. How would this benefit me?
The Plan has the financial benefit of spreading the costs over a two-year period. If you are not based in the UK, your annual appointment can be completed either in person or we can arrange a video consultation, if required.
Will there be a translation/interpreter services included within the Plan?
No. However, if you let us know in advance that you will require an interpreter, we can arrange one on your behalf. The costs of these services are additional to the Plan.
Is the Plan designed for insulin dependent or non-insulin dependent type 2 patients?
The Plan is open to all people with type 2 diabetes, but if you are treated with multiple insulin injections – especially if you have a varied lifestyle – our type 1 Care Plan for people on multiple daily insulin injections or pump therapy might be better suited to you
Can I have the appointments upon request or is there a set time frame?
You can schedule the appointments at a time that suits you, however it is recommended that appointments are scheduled at regular intervals throughout the year.
Who will call me?
You will be hearing initially from our specialist diabetes team coordinators who will liaise with you to arrange your appointments. Your telephone consultations will be taken by a consultant diabetologist, diabetes specialist nurse or a dietitian.
Are there any other benefits or discounted offers?
Yes, the Plan includes a discount on additional services (see London Medical Services information sheet). Furthermore, as a Plan member you will be entitled to a 10% discount on all purchases from the pharmacy.
What if I need additional appointments? What would the cost be?
You will have already had a full assessment and detailed consultation with a specialist consultant before you start the Plan. They will be able to assess your likely requirements and will have recommended the Plan if they are confident that it will deliver what you need with respect to your on-going care.
If you do require any additional appointments or tests then we will plan these with you in addition to the Plan. Illustrative costs of most frequently requested services are listed on the accompanying tariff sheet.
