The London Diabetes Care Plan is different in that we want to recruit you into our expert team as a central player in the decision-making. Self-management shouldn’t mean self-supporting, and we know that confident self-management progresses from being part of a dynamic and collaborative group.

We’re a dedicated team of specialist consultants, dieticians, and diabetes specialist nurses, who will work with you to enhance your quality of life, and offer greater freedom to enjoy it to the full. Working together on ‘Project You’, we’ll be equipping you with the very latest in diabetes care knowledge and technology specifically tailored to your needs. We won’t just be looking at your condition medically, but also looking in-depth together at other parts of your life that may inform your condition.

Options/additional services:

Patients may want to top up or add to their Care Plan, which can be discussed with the team once registered on the Plan. These optional extras can include regular video consultations, continuous glucose monitoring, and special tests on liver, sleep apnoea, and biochemistry.

How does it work?

The London Diabetes Care Plan is a continuous care scheme that means the patient has constant support in managing their diabetes. We have a dedicated team of experts for you to call upon at any time in addition to the defined benefits of the scheme. The minimum term of plan membership is two years. If you require extra services you can select these with the help of your support team (see tariff sheet).

What does it cost?

Two payment options are available:

You can book for a screening appointment where we can assess your health and judge if you are suitable for the care plan, this costs £99 (see ‘Where to next’ diagram above).
Secondly if you have already been screened elsewhere and are looking to receive a second opinion then you can book a consultation with an expert diabetologist which costs £250; this cost will be deducted from the cost of the full care plan if you decide to go ahead.
